"Patient Experience Project" Video Campaign
Share your story and empower others.
Purpose:
To gain a better understanding of what shared decision making looks like to a patient.
To allow patients to share their experience with shared decision making with others in the hopes that these video stories will empower others to engage in shared decision making.
Share your story:
This is your chance to describe how you took an active role in your medical care and used shared decision making to arrive at the right decision for you. We know that individuals facing medical decisions need support; by sharing your story you will inspire others to take charge of their own health.
We have created a video ask containing two examples of shared decision making stories, one from a patient and one from a provider. So now it is your turn. We encourage you to submit a 30-90 second video that answers the question: What does shared decision making look like to you?
How to share your story:
- You must have a YouTube account to upload a video. If you do not have one, you can easily create an account and then share your story.
- Click here to open this video on YouTube (will open in a new window).
- You will see a comment box below the video, click in this box and then click "Create a video response" (located on the right).
- Upload your video! Tag your video with #FIMDM and #PEP.
